Setup Process (6 Easy Steps):

1. After your lab fee is paid, you will receive a link to watch a brief introduction video. A code will be provided at the end of the video.

2. Email the code to: info@lifelinehealthinstitute.com.
   Your order will not be processed until the code is received.

3. Within 72 hours, you will receive:

   • Your lab pricing sheet & agreement

   • A link to register for the National Drug Testing Directory

4. You will receive a link to set up your billing profile with the lab.
   A valid form of payment must be kept on file. This is how you will be charged for each drug test you perform.

5. Once your payment method is on file, your supplies will ship and arrive within 10 business days.

6. You will have access to your lab portal and testing resources to begin offering services.
